An incident doesn’t just test the safety program. It tests the record behind the safety program. Once someone is hurt or a serious near miss occurs, the questions come quickly. Was the worker trained? Was the training current? Did it apply to the task? Were supervisors trained? Were refreshers assigned after previous incidents or inspections? Can the employer produce the records without scrambling?
For many organizations, the problem isn’t that no training happened. It’s that the proof is incomplete, scattered, outdated, or difficult to connect to the work being performed. A sign-in sheet may show attendance, but not comprehension. A certificate may show completion, but not whether the content matched the hazard. A spreadsheet may exist, but it may not show the audit trail leaders need after an incident.
